Abstract
Grahaṇī Roga represents a classical Ayurvedic disorder characterized by impaired digestive function and chronic malabsorption, primarily affecting the duodenum and proximal small intestine. Despite being described millennia ago in major Ayurvedic texts including Caraka Saṃhitā, Suśruta Saṃhitā, and Aṣṭāṅga Hṛdaya, its diagnostic principles show remarkable correlation with modern approaches to malabsorption syndromes. This comprehensive review synthesizes classical Ayurvedic diagnostic principles including Nidāna (etiology), Pūrvarūpa (prodromal features), Rūpa (clinical symptomatology), and Parīkṣā (examination methods) with contemporary investigative modalities used in diagnosing tropical sprue, celiac disease, and related enteropathies. The Ayurvedic diagnostic framework provides holistic, constitution-based assessment through Aṣṭavidha Parīkṣā (eight-fold examination) and Agni evaluation that complements modern structural and biochemical investigations. Doṣa-specific symptomatology correlates with distinct pathophysiological patterns observable in modern gastroenterology. Integration of both systems enables comprehensive evaluation, improved diagnostic accuracy, and personalized management strategies. The combined diagnostic approach utilizing Ayurvedic clinical examination with modern serological testing, endoscopic evaluation, and histopathological analysis offers superior diagnostic capabilities for malabsorption disorders, enabling early detection, accurate classification, and individualized treatment strategies. This integrative model represents a template for bridging traditional and contemporary medical systems while serving the best interests of patients suffering from chronic malabsorption disorders.
