Abstract

Penile strangulation is a rare but potentially severe urological emergency requiring prompt management to prevent complications such as tissue necrosis, urethral fistula, or loss of penile function. We report the case of a 41-year-old man presenting with acute urinary retention and penile lymphoedema secondary to the presence of three metallic rings at the base of the penis. Management required urgent intervention under locoregional anesthesia, with ring extraction using a metal-cutting disc. Despite initial improvement, a penoscrotal urethral fistula developed, necessitating secondary surgical management. This case highlights the importance of rapid and multidisciplinary intervention in managing this condition.
