Abstract
This project presents a comprehensive web-based Electronic Health Record (EHR) System with a dual-portal architecture designed for seamless collaboration between healthcare providers and patients. The AI-Driven Electronic Health Record System for Smart Diagnosis and Care modernize healthcare management through secure, efficient, and intelligent digital
solutions. It distinctly serves both doctors and patients, ensuring that each user group has access to features tailored to their specific needs. Doctors benefit from a comprehensive dashboard that aggregates essential clinical metrics such as appointment schedules, patient loads, and monthly activity statistics, along with integrated tools for prescribing medication, viewing patient reports, and managing communication. The patient portal, in turn, empowers individuals to actively manage their healthcare journey by booking appointments, tracking ongoing medications, viewing medical history, requesting prescription refills, and securely reviewing their overall health status.
