Abstract
The incident reporting system is designed to provide critical information about patient safety, with the goals of reducing risks, improving systems, and facilitating learning from mistakes and various lessons learned. One key factor in the success of incident reporting is the system’s ease of use. This research aims to evaluate the implementation of the patient safety incident reporting system at TMC Tasikmalaya Hospital, employing a mixed-methods approach. Quantitative data were gathered from patient safety observations between January and March 2023, patient safety culture measurements in 2023, and patient safety incident reporting data for the same period, while qualitative data were obtained through interviews with informants about patient safety implementation and the evaluation of a web-based incident reporting application. Data collection techniques included in-depth interviews, documentation, and observation. The findings revealed that 54 unexpected incidents, 64 non-injuring incidents, 171 near-injuring incidents, and 47 potentially significant injuring incidents were reported. To facilitate easier reporting, researchers developed an electronic-based incident reporting application, which is expected to streamline the reporting process. Continuous outreach and training by the Sub-Quality Patient Safety Unit to healthcare staff in the care units are necessary to enhance the application’s effectiveness and improve patient safety reporting.
